Fit Note Requests

Fit notes, sometimes referred to as sick notes or medical certificates, are issued by qualified healthcare professionals to document your fitness or ability to work. These notes are usually required for your employer when you need to explain absence due to illness or if you require adjustments to your duties or working conditions.

For sickness episodes up to seven days, employees can normally self-certify and do not require a fit note. If illness continues beyond seven days, a formal fit note is needed and is issued after a clinical assessment by a healthcare professional. Private fit notes are available, but it is best to check with your employer in advance to make sure they accept them.

Fit notes are typically issued for a period of up to 14 days. Extensions may be possible in certain circumstances, such as after an operation and with appropriate supporting documents, though this is not routine practice. Fit notes are generally dated for the day of assessment and rarely backdated; they cannot be future dated to start and reflect the clinical situation at the time of assessment.
