Complete Pre-Consultation Forms
After booking, you will receive forms to fill out with your details, including:
- GP information
- Medical history and allergies
- Identification
Please note
For children’s consultations, a parent or legal guardian must be present. You will also need to upload the child’s birth certificate and the parent/guardian’s ID.
Additional Information
(if required)
For certain conditions, we may send you an additional form to help the doctor prepare and make your consultation as smooth and effective as possible. There is also space to share any extra information you’d like the doctor to know.
Preparing for Your Appointment
Please ensure you join your video consultation from a private workspace or quiet location with a reliable internet or phone connection. Avoid attending from public places, trains, or while driving. Privacy and a stable connection will help ensure your consultation is smooth, confidential, and effective.
Attending Your Appointment
Please join the video consultation a few minutes before your scheduled start time. Your doctor will join you shortly. We aim to run on time, and if there is any delay, we will contact you by phone.
After Your Consultation
You will receive a summary and a management plan outlining any investigations, referrals, prescriptions, or follow-up actions. With your consent, we can also update your NHS GP.
